For our regular patients(those we have seen within 12 months.)

For new patients, or patients we have not seen in more than a year.

Please call the clinic if you need a refill and provide your weight, blood pressure and pulse. If you are unable to provide your blood pressure and pulse, it may be waived by the provider.

At the end of the Telephone visit, you will be asked to provide a credit/debit card information for payment. After the payment you can come to the office to pick up your medication.

Wednesday 8:30 am - 4:00 pm

Saturday 8:30 am -12 pm
